We do alot of overseas shipping and Guam is not a part of the US.
If the USPS requires a "Customs Declaration" form the shipping destination is outside the US.
If you do not wish to ship outside the US, I would advise emailing the buyer, pointing out the no outside US shipping, and mutually cancel the sale.
If you wish to go ahead with the shipping, I would advise you put the correct totals on the form and let the buyer worry about paying any Customs taxes necessary.
I have had international buyers ask me to put fraudulent totals on the form in order to avoid taxes or to mark it as a 'gift' when it is not. Doing so is against the law and will get the seller in big trouble with the Postal Inspectors.

No, you don't need to fill "Customs Declaration", Guam is considered domestic mail with USPS because it is a U.S. territory.

It is a US Territory and the US Post Office considers this a "domestic destination" (DMM 608 2.1) BUT customs forms are required in some cases:

2.4 Customs Forms Required
2.4.1 Priority Mail Weighing 16 Ounces or More

[11-2-09] Regardless of contents, all Priority Mail weighing 16 ounces or more sent from the United States to ZIP Codes 96910-44, 96950-52, 96960, 96970, and 96799, and all Priority Mail sent from these ZIP Codes to the United States, must bear customs Form 2976-A. If Form 2976-A will not fit on the address side of the mailpiece, Form 2976 must be used. This mail must be presented to an employee at a Post Office, to a letter carrier when using Click-N-Ship with Carrier Pickup, or to a Postal Service employee designated by the postmaster.

I ship to Puerto Rico with out any customs form and my buyers get their package promptly. From what I see on USPS's website that goes for GUAM as well.

You could always call the USPS and ask their customer service they are very friendly there.

I sent my husband to Post office and was told him that I have to fill 2976-A form.
Yes, the buyer's zip code is 96913.
I don't know the exact weight, I think it weighs less than 1 lb.
It's real eye opening experience.

I've shipped to Tinian, MP 96952 several times without customs forms. try print out the shipping label from paypal. you can ship with priority mail flat rate envelope. it only costs you $4.80 with DC.

Although I specified shipping only within USA, the buyer actually into the auction without mentioning me where she lives.
Anyway, I paid extra $5 my money into medium flat rate durable packing box instead of an envelop (not breakable ) from Paypal.
Oh well, I am a new and it's; learning curve....

Let's get this straightened out once and for all.

Go to the USPS online rate calculator. (Link) Select "Guam" as your destination.

Are you directed to the international shipping calculator or the domestic shipping calculator?

For international shipping you'll be given four "shapes" to choose from and you won't need to specify ZIP codes. For domestic shipping, you'll be given five shipes to choose from and you will have to specify ZIP codes.
